The face of medicine is changing,, the little guy like me in private practice is a struggle anymore what with all the new regulations and endless paper work,, there are more and more self pay Medicaid and Odipshietcare patients coming out of the woodwork everywhere,, the expenses are going up and and the money is going down,, over half of the patients at my small hospital are self pay so you treat for free,, Medicaid pays little and Obamacare is a total mess,, however, the govt is going to try and save these poor souls with there new approach to providing substandard care,, guess its better than nothing but you see what happened with the VA thing,, that will be a mainstay with Obamacare,, and I'm right in the middle of all this this so I have the knowledge to see this and I have been warning folks for years on this board,,
I for one have changed my whole practice to stay afloat,, I've given up the hospital practice and stopped working the ER,, the ER is the worst,, people are demanding and in many cases you just cant accommodate them the system is not there,, I for one am in it for the money now,, I'm doing regional anesthesia and have a weight loss clinic,, many pay cash,, in ten years from now, stem cells will be the mainstay in treatment of disease and that is a good thing,, good things on the horizon, but for me it is a matter of survival and I sure as shiet don't want to be a hospitalist like many are going to,, but, that is what your going to see,, docs going to work for hospitals cause you cant make it in private practice the traditional way, so I'm in it for the money,, let the govt provide for the poor,, I've been doing it for 25 years and in the last couple of years have lost my a()ss,,
Here is what you have to look forward to,, small clinics popping up all over the country, staffed with nurses and PA's,,
Your post was an interesting read! Coincidentally, I left my hospital-owned IM practice this year and joined a nearby private practice. They don't take medicaid plans and no at-risk medicare advantage plans. I was nervous making such a drastic move from the hospital, but it looks like it will be the right move. I am honored and humbled by the number of loyal patients who have left the previous practice to follow me. Sad to leave behind many nice people whose insurance is no longer compatible with the new office.
I think PCPs have been brainwashed to believe that they need the hospital business structure to survive. What a relief that I no longer have to pay for overhead of multiple layers of administrators and businessmen salaries at the hospital. They attend meetings all day and conclude that the doctors, PAs, and NPs should work harder! The support structure should exist to assist the doctors to help the patients. Presently, many doctors feel enslaved that the hospital support structure that has entangled them. Doctors need to rethink the situation: it is really the hospital administrators/ businessmen who need the doctors, not vice versa. If you are not seeing a patient or operating on a patient or counseling a patient or booking a patient, or doing something that directly assists a patient.....then, you must be overhead!
From all that I can figure medically, ACTC has real and significant potential to help patients and reward shareholders, and we are going to witness significant growth in value very soon!

Good to here from you,, I think you did the right thing,, lot of docs are doing just what you did,, joining the larger practices,, unfortunately in my small town no one is interested in doing so,, we have 7 docs in our small town,, 2 of which are retiring,, I've worked ER and ICU for 25 years and are both going to miss the intensity and the same time I,m sick of not getting paid,, in our hospital, they do not pay us,, we get what we collect and there are many self pays,, no, I have to change my plans in order to make it,, I own my own building, which is the nicest in town and thinking of renting it to the hospital and take a smaller office,, I was trained early on by my mentor in regional anesthesia and just upped my malpractice to do blocks,, I'm even considering a course in botox,, mostly cash money,, I still plan to work ER once or twice a month to keep up my critical skills,,
Many are going to concierge medicine,, that puts me out in a small town,, the cities will do this and this sounds a little of what you are doing,, again, I think you did right,,

I don't care about me or my family,, we will be fine,, I care about the poor slobs that work and pay taxes and have to buy into this garbage and pay out the nose for limited care,, my patients are suffering because they cannot get testing, medicines or care that they need and deserve as working folks,, the one's that don't work to live off the system and are capable, I could care less about and do not go out of my way to help them period,, ya know, the jerks that want free stuff and not work for it,,
