Part of the story (probably a big part) is that victory over CIN has been declared before, more than once. I'd expect the Italian market to become 'interesting' by year end if the results continue as they are. Because [outside the US] procedures involving large amounts of contrast medium tend to be done in larger centers, console sales won't be as big a deal as disposable kit sales.
Rememmber that most of the countries of Europe have to be considered separate markets for medical devices. It's reasonable to expect sales in 'going' markets to get substantial while nearby countries have [almost] zero use.
