checkpoint inhibitors only work if cancer cells are expressing mutant proteins that serve as antigens so the immune system reacts. Most - vast majority - of tumors do not express antigenic proteins on the cell surface capable of eliciting an immune response. This is how cancers become established - they evade the immune system and appear to be normal cells. Melanoma is an exception and is highly mutated. There are instances of "spontaneous" remissions in metastatic melanoma patients that are now being attributed to immune system recovery following cessation of standard chemo treatments that suppress immune responses.
Some of the ideas being discussed to make checkpoint inhibitors more effective are laughable if it weren't so sad. Some oncologists think treating tumors with genotoxic agents (mostly old-school chemo) will cause more mutations so the immune system will recognize the cancer as being foreign.

IMO approval is no-brainer. impact on SP is, as you suggest, completely unpredictable. I very much doubt approval is "baked in". IMO SP is manipulated and will continue to be so until BigPharm makes a move to buy. Only then will true value be reflected in SP.
CAR-T has been way oversold. It's been incredibly effective in extremely limited number of indications - mostly B cell lymphomas. Entire technology depends on unique antigens expressed exclusively on surface of malignant cells. A vaccine approach would work with such a scenario; except where CAR-T has been successful - a vaccine approach requires B cells to make antibodies against the cancer cells. CAR-T therapy targeting CD20 or CD19 eliminates any and all B cells - even non-malignant B cells. "Cured" patients are immunocompromised without B cells but effective therapies exist to deal with this. Genetically engineering an immune response is extremely complicated stuff and very experimental. As is the case with immune check point inhibitors, but antibodies against check point inhibitors don't require genetic manipulation of patients immune system.
Immunotherapy for cancer is in earliest stages and insurance companies do not pay for experimental therapies. Small molecule therapeutics that effectively inhibit cancer progression with manageable side effects and maintain quality of life are the "new" standard in recent years and will continue to be SOC for foreseeable future.
