    How do you arrive at $375? It would help if any guesses also identify how guess was determined.
    CMS doesn't just pull a number out of the air as they already indicated that number will not be determined based on cost effectiveness.

    2 DNA methylation markers are $282.83 each
    7 DNA mutation markers add up to $197.48
    Fecal Hemoglobin is $21.70
    Grand total is $784.84

    EXAS is already giving discount of $282.83 since they are only charging for 1 DNA methylation marker when they are using 2. Grand total becomes $502.01

    I don't believe CMS will make them discount further so my guess is $502.

    Switch from Colonoscopy to Cologuard

    From Medscape article today:

    "Another clinician went as far as to suggest, in an editorial published this week in JAMA (2014;312:601-602), that Cologuard might someday eclipse both colonoscopy and sigmoidoscopy as the initial screening test of choice in the United States."

    "Stool DNA testing has the potential to reduce colorectal cancer mortality substantially while sharply reducing the number of routine colonscopies," summarized Allan Brett, MD, from University of South Carolina in Columbia.

    Switch from Colonoscopy to Cologuard

    I agree, once a patient is educated about advantages of Cologuard, they will more often refuse Colonoscopy. I can't envision many doctors letting the patient walk away without offering Cologuard as an alternative. Even if they are not completely on board with Cologuard, it is better than their patient getting NO screening.

  • The percentage of folks not choosing any screening currently is a known number.
    The big unknown is the number of folks that will opt for Cologuard that would have screened with Colonoscopy.
    I think this number will be huge. Folks with any concern/fear will opt for Cologuard.
    Folks like myself due for 2nd Colonoscopy will opt for Cologuard. Even my own Physician nearing
    50 said he would opt for Cologuard.

  • You hit the nail on the head - "seem like they don't care". Physicians (mine included) do not like the FIT test and are reluctant to recommend it for patients refusing Colonoscopy. They just keep pushing Colonoscopy even when patient does not want or follow through with it. Physicians will be much more likely to recommend Cologuard for their patients who do not want a colonoscopy. I believe their enthusiasm will be passed on to the patient. Once patient understands how simple and accurate this new test is, they will prefer over Colonoscopy.

    Also, have you ever done a FIT test? I have and it actually requires 6 interactions (6 card circles) with stool over a recommended period of 2 days. Cologuard will be much simpler with a single interaction with stool from the container instead of fishing around for floating samples in toilet water. I had a colonoscopy at age 48 and nothing found. I will now request Cologuard as I am coming up on 10th year after my Colonoscopy. I will be more than satisfied to do Cologuard every 3 years going forward.
