NASHVILLE, Tenn. & PITTSBURGH--(BUSINESS WIRE)--

naviHealth and Highmark Inc. have announced a three-year partnership to manage post-acute care services for Highmark’s Medicare Advantage members across Pennsylvania. The program will utilize naviHealth’s unmatched decision support technology, post-acute analytics platform and network of field-based clinicians to enable Highmark members to achieve improved outcomes following a hospital stay. With more than 285,000 members across Pennsylvania, Highmark is currently the largest Medicare Advantage plan in the state.

“This service will ensure that our members are receiving the right care in the setting that is most appropriate for their continued recovery,” said Donald R. Fischer, M.D., Highmark's chief medical officer. “This partnership is another way Highmark is serving as a leader and differentiating ourselves by being the first in our market to offer this innovative care delivery approach.”

On average, more than 40,000 Medicare beneficiaries are discharged from hospitals every day. Many leave the hospital without a coherent plan or guide for what’s next. This leads to large variations in post-acute care utilization, spending and outcomes. naviHealth aligns stakeholders through a unique field-based services model and data-driven approach leveraging the largest post-acute patient outcomes database of its kind.

“We are excited to work with Highmark to help optimize post-acute care for their members and ultimately deliver improved outcomes through our high-tech, high-touch approach,” said Clay Richards, naviHealth’s president and chief executive officer. “By any measure, Highmark is recognized as one of the nation’s leading health insurers. This partnership represents not only an important moment for the evolution of naviHealth’s business, but also another indicator that leading health plans are seeking innovative ways to address the practice variation and rising costs that exist in post-acute care.”
