DALLAS, TX and EUGENE, OR--(Marketwired - Jun 19, 2013) - Phytel, the leader in automated, provider-led population health improvement, announced today that Oregon Medical Group (OMG), a primary care-based multispecialty group with more than 110 providers in Eugene and Springfield, Ore., has selected Phytel's comprehensive, Web-based population health management suite as a critical component of its strategy to build new healthcare delivery models.
Oregon Medical Group will use Phytel's Outreach, Insight, and Coordinate modules to ensure that every patient receives the most appropriate intervention and follow-up care. Additionally, OMG will introduce Phytel Transition to improve care coordination for patients who are discharged from the hospital, and to reduce preventable readmissions.
Key to the medical group's decision to deploy Phytel was its role as one of a select group of physician practices nationwide to participate in a program designed to improve care and care coordination for patients. Oregon Medical Group joins a select group of practices as a participant in the CMS Centers for Medicare and Medicaid Innovation's Comprehensive Primary Care Initiative (CPCI).
"To succeed under CPCI, our providers and care managers need advanced tools to improve the health of all patients, especially those who require more care to stay healthy and out of the hospital," said Josh Kimball, Chief Operating Officer of OMG. "That's a tall order but now with Phytel our providers and care managers will have access to a full suite of automated population health intelligence to eliminate variation, increase patient engagement, improve care coordination and keep people healthy across the continuum of care."
Phytel CEO Steve Schelhammer said, "We are committed to helping Oregon Medical Group succeed within the CPCI program, which is a microcosm of the overall movement toward value-based care in our healthcare system. With the assistance of our user-friendly care coordination platform, expertise in patient engagement, and knowledge of population health management, we're confident that Oregon Medical Group will achieve its goals."
Phytel's Population Health Management solution aggregates data from electronic health records and practice management systems in hundreds of physician groups nationwide and offers a suite of tools for automating population health management, including modalities for patient outreach, care gap identification, risk stratification, care management, patient engagement, transitions of care, and performance evaluation.
Oregon payers participating with Medicare in the CPCI program are CareOregon, Oregon Health Authority, Providence Health Plans, Regence BlueCross BlueShield, Teamsters Multi-Employer Taft Hartley Funds, and Tuality Health Alliance.
CPCI practices were selected through a competitive application process based on their use of health information technology, ability to demonstrate recognition of advanced primary care delivery by accreditation bodies, service to patients covered by participating payers, participation in practice transformation and improvement activities, and diversity of geography, practice size and ownership structure. It is a credit to Oregon Medical Group's quality of care that they were selected to participate in this initiative.
The premier company empowering physician-led population health improvement, Phytel provides physicians with proven technology to deliver timely, coordinated care to their patients. Phytel's state-of-the-art registry, which now encompasses more than 30 million patients nationwide, uses evidence-based chronic and preventive care protocols to identify and notify patients due for service, while tracking compliance and measuring quality and financial results.
