As of April 2, 2015, the company had 144 facilities, 11 hospice companies, 13 home health businesses, 2 home care businesses, and 16 urgent care clinics in California, Arizona, Texas, Washington, Utah, Idaho, Colorado, Nevada, Iowa, Nebraska, Oregon, and Wisconsin.
