Conviva Expands Healthcare Accessibility to Texas Seniors with Upcoming Openings of Three New Care Centers in San Antonio

·4 min read

‘Senior-focused primary care’ provided at each new center, available to all people who are eligible for Medicare

SAN ANTONIO, October 28, 2021--(BUSINESS WIRE)--Conviva, a physician-led network that specializes in senior-focused primary care, is expanding its reach in the San Antonio area by opening three additional Conviva Care Centers here.

Grand opening celebrations will take place at each center in November at Conviva Medical Center, 7959 Fredericksburg Road; Conviva San Pedro, 7142 San Pedro Ave.; and Conviva Thousand Oaks, 4455 Thousand Oaks Drive. The new centers will bring the total number of Conviva Care Centers in the San Antonio area to 11.

Approximately 10,000 Baby Boomers turn 65 each day, and about 44 percent of Texans are 60 or older. Conviva’s new centers will help meet the health care needs of the senior population in San Antonio, offering a range of whole-person health services.

Wes Smith, president of Conviva’s San Antonio operations, said senior-focused primary care requires specialized knowledge, along with compassion and the ability to recognize nonmedical factors that might affect someone’s health. "Our centers are led by physicians who take the time necessary to work with each patient. We don’t rush care," Smith said. "We work to make sure we’re addressing everything that might keep you from achieving your best possible health."

The grand-opening open houses will offer caregivers, patients and potential patients a convenient opportunity to tour the facilities, meet the Conviva physicians and other members of the care team, and gain a better understanding of the services available through the Conviva network.

James and Carol Armstrong of San Antonio say they like the way they’re treated at Conviva. "Every three months, we have a wellness/maintenance check," the Armstrongs said. "They take your tests, medications and compare with the previous three months, then they adjust from there. You would not find another group focused as much on mental, physical and emotional health."

Conviva operates more than 100 care centers in Texas and Florida. Conviva Care Centers accept a variety of Medicare and Medicare Advantage health plans.

Conviva is a wholly owned subsidiary of Humana, which has a longtime history in the city of San Antonio and a strong commitment to improving the health of San Antonians. San Antonio was the first of 16 cities to become a part of Humana’s Bold Goal, an ambitious population health strategy launched in 2015 that focuses on addressing the social determinants of health and the health-related social needs of Humana members and communities.

Besides offering the checkups, diagnostics and treatments available traditionally available in conventional doctors’ offices, Conviva Care Centers also address external factors that research shows account for up to 80 percent of a senior’s wellbeing. These social factors may or may not include food insecurity, lack of transportation, inadequate housing and issues related to the patient’s mental health. Depending on the patient’s needs, the Conviva care team can help resolve such issues in-house or through referrals to other community resources.

Conviva clinics also include activity centers where seniors can socialize, take health-education classes, participate in arts activities and even exercise.

As with other Conviva centers, office hours for the new clinics will be 8 a.m. to 5 p.m.—and same-day appointments are often available. Even after hours, or on weekends, callers can always talk with a live person who will connect them to an on-call doctor.

Each grand-opening event will offer food, entertainment, giveaways and tours of the facility from 10 a.m. to noon. More details:
